Our goal is to provide support and coordination of services for Cowlitz Indian Tribal Members—advancing the Enrollment Department’s mission, vision, and values through efficient, proficient, and effective member services.

Call Option 3 or email Enrollment with the required information to request an application.
Submit & Follow Up
Provide required documentation and follow up with Enrollment if additional information is needed.
Cowlitz Indian Tribal Enrollment Criteria
All enrollment applicants must meet all criteria below to request an enrollment application.
- 1Lineal Descendant
Applicant must be a direct lineal descendant of a Cowlitz Indian.
- 2Parent on Roll
Applicant must have a parent (mother/father) living or deceased on the Cowlitz Tribal Roll.
- 3Age Requirement
Applicants must be between the age of 0–18 years old (must apply before your 19th birthday).
Request an Enrollment Application
Request an enrollment application by calling 360-577-8140 (Option 3), or email your request to enrollment@cowlitz.org.
Include: full name, date of birth, Tribal parent name, and address.
Services & Programs
Photo Tribal ID Cards
Email a clear headshot picture + your mailing address to enrollment@cowlitz.org.
Elder Vests
Members 60+ can receive one free vest. Call (360) 577-8140 (Option 3) to request an application.
Burial / Funeral Assistance
Members are eligible for up to $4000 for themselves or a spouse.

Non-Resident Rule
- A non-resident Washington State license through the Tribe, OR
- Reimbursement for an out-of-state license through Enrollment.
Reimbursement Steps
Email receipts to enrollment@cowlitz.org and include:
- Full legal name
- Date of birth
- Cowlitz Enrollment Number
- Photo of license showing type, cost, date purchased, and time period covered
Covered Licenses List
- Annual Combo Fishing / Shellfish License
- Freshwater License
- Annual Razor Clam License
- 3-Day Combination Fishing License
- Deer + Elk + Bear + Cougar (Hunter Safety required)
Uncovered items include endorsements and reprints.
Enrollment Update
6/3/2025 — NEW MEMBER PORTAL. We are excited to announce the launch of our new Member Portal. As part of this update, you are able to update your contact information and request new ID cards and more.
On 6/3/2025 and 6/4/2025 members received an email with instructions to create their Member Portal account. Once registered, you'll be able to update your enrollment information and explore additional features and resources available.
